Visa Application Form All visa applicants (regardless of age and visa category) are required to submit an electronic visa application form DS-160. At the time of the interview, applicants must submit the printed confirmation page with the bar code. The DS-160 replaces DS-156, DS-157 and DS-158 application forms. For further information and to complete the DS-160, please visit https://ceac.state.gov/genniv. The form is also accessible on the Embassy’s website, under “Visas to the U.S.” at  www.usembassy.de. In addition to scanning to the DS-160 application a photo according to the standards, you should bring it to the interview. IMPORTANT INFORMATION FOR STUDENTS AND EXCHANGE VISITORS: The DS-160 online form requires “J” (exchange visitor, au-pairs), “F” (academic student), and “M” (vocational student) applicants to enter their SEVIS number and school information. Sponsoring organizations should provide their students with the information about SEVIS numbers and payment.
